Our on-site medical group is exclusive to our members and allows residents to meet with on-site health specialists and clinicians, work with physical therapists, and even pursue holistic therapies. Find peace of mind, knowing that your health and wellness is our top priority.
A provider is available by phone 24/7 and telehealth can be utilized to meet immediate needs.

Wellness Center
For your convenience, our on-site wellness clinic can serve as your primary care provider (PCP) so you never have to leave the community to see a doctor. If you already have a PCP, you can still use our services any time. We’ll also coordinate with your specialists, including physical, occupational, or speech therapists. We're here for you, quite literally.
Stephanie White, FNP-C
Stephanie became a family nurse practitioner in 2015. She has experience working in telemedicine, PCU, ICU, independent living, assisted living, long term care, rehabilitation, and hospice. She received her Bachelor of Science in Nursing degree from Research College of Nursing and her Master of Science in Nursing degree from Maryville University, graduating as a board-certified family nurse practitioner. She is also certified by the AANP and AMNP. She chose to become a nurse practitioner and focus on caring for seniors because she enjoys getting to know her patients on a personal level. She strives to build trusting relationships with her patients in order to foster a supportive and comfortable environment allowing her patients to feel heard and understood.
Sarah E. Hugo, MD
Dr. Hugo began practicing internal medicine in 2011, initially at a community-based primary care practice at Mayo Clinic in Rochester, MN. In this role, she cared for a diverse patient population and also had the privilege of teaching resident physicians. My educational journey includes earning BA degrees in Biology and Chemistry from Drury University in 2004, followed by graduating with an MD from the University of Missouri-Columbia School of Medicine in 2008. Dr. Hugo completed her internal medicine residency at Mayo Clinic in 2011 and is board-certified by the American Board of Internal Medicine. Building personal connections with patients while addressing their medical needs brings her immense fulfillment.

Of course! Our on-site medical clinic providers are available to see you whether we are your primary doctor or not. However, if you decide that you want to switch to our providers for primary care, that is always an option.
Absolutely! You can think of the clinic as a regular urgent care. Walk-ins are always welcome, or you can call to schedule an appointment.
Yes! We understand that our patients are not always able to physically come into our clinic, so you can call to schedule an in-home visit!
It’s what we do. At our on-site medical clinic, our goal is to provide comprehensive care for our patients, so we coordinate with any doctors or specialists that you may see to make sure you’re receiving the best care.
Yes. Our providers will always send any visit notes with doctors, including your primary care doctor, to make sure everyone is up to date on your care.
